Request for Leave of Absences that fall under the Family and Medical Leave Act (FMLA) ​go through Prudential:

Reporting Leave of Absence or Disability

Submit requests online at www.prudential.com/mybenefit, email the LOA email [email protected], & call Prudential to confirm all information (877) 367-7781 their office hours Mon-Fri 8AM-8PM:

• All LOAs are 7 days or more must be filed through Prudential 
• Prudential will process and track LOAs. They will notify the park and Corp.
• Communication to the employee will come from Corp and Prudential
• The Park will be responsible for any accommodation approvals and communication on pay
